By Signing my athlete(s) up for sports through North Sevier Recreation, I agree to be a positive example of good sportsmanship and show full respect to:
-The Current Rec Director (Devyn Storey)
-The Referee’s, regardless of experience
-My Athletes coach
-The opposing teams coach
-The opposing teams parents
And any other individual involved in the program that may not be stated.
I understand that I may be permanently removed from viewing or participating in my athlete’s recreational activities if the above statement is not followed.
Youth Sports Code of Conduct Agreement
This Code of Conduct Agreement is designed to ensure a positive, respectful, and safe environment for all participants in the North Sevier Recreation youth sports program. By signing this agreement, all parties agree to uphold the standards and expectations outlined below.
For Athletes
As an athlete, I agree to:
Show respect to all teammates, coaches, officials, opponents, spectators and any others in association of the North Sevier Recreation Youth Sports Program.
Play fairly, following the rules and showing good sportsmanship at all times.
Attend all practices, games, and team meetings punctually, unless I have informed my coach of an unavoidable reason for my absence.
Listen to and follow my coach’s instructions.
Avoid using foul language, taunting or engaging in physical altercations.
Report any incidents of bullying, harassment, or inappropriate behaviors to my coach or another trusted adult.
For Coaches:
As a Coach, I agree to:
Treat all athletes, parents, officials, opponents and anyone else associated with the Youth Sports Program with respect and fairness.
Serve as a positive role model by demonstrating good sportsmanship, integrity, and ethical behaviors.
Create a safe and inclusive environment where all athletes feel valued and supported.
Educate Athletes about the importance of teamwork, discipline, and respect.
Address any issues of bullying, harassment, or inappropriate behaviors promptly and effectively.
Communicate clearly with athletes, parents and the Recreational Department about expectations, schedules and any changes.
Will refrain from any aggressive physical contact or behavior towards athletes or parents.
For Parents and Guardians:
As a parent or guardian, I agree to:
Encourage my child to play by the rules and respect the efforts of the coaches, opponents, officials and the Recreational Directors.
Support my child’s participation in practices and games, and other team activities by ensuring they arrive on time and are prepared to participate.
Refrain from coaching my child or other players during games and practices from the sidelines.
Communicate any concerns or questions to the coach or Recreational Department in a respectful and constructive manner.
Promote a positive and supportive atmosphere during games, avoiding negative comments or disruptive behavior.
Report any incident of bullying, harassment, or inappropriate behavior to the Coach or Recreational Department.
Consequences of Violating this Code of Conduct:
Violation of this Code of Conduct may result in one or more of the following actions, depending on the severity and frequency of the offense:
Verbal warning from the coach or North Sevier Recreation Department
Written warning and meeting with the coach, athlete, parents and the North Sevier Recreation Department.
Suspension from practice games or team activities for a specified period of time.
Expulsion from the team.
Permanent Removal from Attending or Participating in any recreational activities.
Referral to the North Sevier Recreation Department for disciplinary committee for further action with possible expulsion from the program.
Agreement:
By signing this document, I acknowledge that I have read, understood, and agree to abide by the code of conduct for the North Sevier Recreational Youth Sports Program. I understand the consequences of violating this agreement and commit to fostering a positive and respectful environment.
Positive Example and Anti-Bullying Policy
This contract aims to establish a clear anti-bullying policy for ALL athletes and coaches. By signing this contract, I agree to abide by the outlined rules and understand the consequences of engaging in bullying behavior.
Zero Tolerance:Any Form of bullying, including physical, verbal and cyberbullying, will NOT be tolerated.
Respect:Athletes must show respect towards teammates, coaches, opponents, and any other individuals involved with the sporting program.
Inclusivity:I agree to promote a supportive and inclusive team environment for ALL members.
Reporting:I agree to report any incidents of bullying to the coach or a responsible authority immediately. (Parents, Recreational Director etc.)
Definition of Bullying:
Physical Bullying: Hitting, Pushing, or other forms of physical aggression.
Verbal Bullying: Name-calling, insults, or other derogatory comments
Cyber-bullying: harassing or threatening messages sent through digital platforms
Social Bullying: excluding someone from activities or spreading rumors.
Consequences for Bullying:
First Offense:
Verbal Warning from Coach.
Immediate suspension from one game
Written apology to the victim.
Second Offense:
Mandatory Meeting with the coach, parents, athlete and Recreational Department.
Suspension from three games.
Probationary period where the athlete's behavior is closely monitored.
Third Offense:
No Refunds Given.
As a North Sevier Athlete, I agree to the following:
I will be a role model for my peers, both on and off the field.
I will exhibit behaviors that reflect positively on myself, my team and my community.
I will not engage in any form of bullying, including physical, verbal, and cyberbullying.
I will promote a supportive and inclusive team environment.
I will report any bullying incidents, whether involving myself or something I have witnessed to the Coach or The Recreation Department.
By signing this contract, the athlete agrees to uphold these standards and understands the consequences of failing to do so.
Concussion and Head Injury Policy
In compliance with House Bill 204 – “Protection of Athletes with Head Injuries Act”; North Sevier Recreation has implemented the following policy which requires adherence by all coaches, volunteers, parents, legal guardians, participants and representatives of North Sevier Recreation, Salina City, Aurora City and Redmond Town.
A concussion is a brain injury, and all brain injuries are serious.
They are caused by a bump, blow, or hit to the head or by a blow to another part of the body with the force transmitted to the head.
They can range from mild to severe and can disrupt the way the brain normally works.
Even though most concussions are mild, all concussions are potentially serious and may result in complications including prolonged brain damage and even death if not recognized and managed properly.
Even a slight bump on the head can be serious.
Symptoms and signs of concussions may show up right after the injury or can take several hours or days to completely appear.
If an athlete reports any symptoms of a concussion or if you notice the symptoms of
concussions yourself, seek medical attention right away.
It is difficult to detect a concussion and most sports concussions occur without loss of consciousness.
Participating in a sporting event after a concussion can leave an athlete vulnerable to greater injury or death.
There is an increased risk of significant damage from a concussion for a period of time after the concussion occurs, particularly if the athlete suffers another concussion before
completely recovering from the first. This can lead to prolonged recovery, or even severe brain swelling with devastating and even fatal consequences.
Concussion Action Plan
What should a parent and/or legal guardian do when a concussion is suspected?
1. Report the suspicion to the coach and North Sevier Recreation Director:
Look for the symptoms and signs of a concussion (see CDC fact sheet)
When in doubt, remove the athlete from play
2. Ensure that the athlete is evaluated right away:
Do not try to judge the severity yourself, get assistance from a qualified Health Care Professional as soon as possible
3. Allow athlete to return to play only with permission from a qualified Health Care Provider:
Repeated concussion prior to recovery can increase the likelihood of further problems
No participant should return to play or practice on the same day a concussion has been diagnosed.
4. Both coach and parent should record the following:
The cause of the head injury and with what force
Any loss of consciousness and for how long
Any memory loss immediately after the injury
Any seizures immediately after the injury
Any other pertinent information you may think will help the Health Care Provider
Acknowledgement
By signing this form and the North Sevier Recreation registration form I acknowledge I have read North Sevier Recreations Concussion and Head Injury Policy.
I understand what a concussion is, have been informed on how to recognize the signs and symptoms and agree to abide by the policy.
I understand if my athlete is suspected of having a concussion, he/she will be removed from the sporting event and will not be permitted to continue participating in any upcoming sporting events until a qualified Healthcare Professional has determined it to be safe.
I will provide North Sevier Recreation with a written statement by a qualified Healthcare Professional acknowledging the athlete is cleared to resume participation.
Equipment and Facility Usage Agreement
As an athlete in the North Sevier Recreation Youth Sports Program, you will have access to various facilities and equipment necessary for conducting practices and games. This agreement outlines the responsibilities and expectations related to the use of these facilities and equipment.
Responsibilities and Expectations:
1. Proper Use of Equipment:
- I agree to use all equipment provided by North Sevier Recreation, including but not limited to balls, nets, cones, and protective gear, in a safe and appropriate manner.
2. Maintenance and Care:
- I will take responsibility for the proper care and maintenance of the equipment while it is in my possession.
- I will report any damaged or malfunctioning equipment to the recreational director immediately.
3. Return of Equipment:
- I agree to return all equipment in good condition at the end of the season or upon request by North Sevier Recreation.
- I understand that I may be held financially responsible for any lost, stolen, or damaged equipment due to negligence.
4. Cleanliness and Order:
- I will ensure that all facilities are left clean and orderly after use.
- I will dispose of any trash and ensure that all personal and team items are removed from the facility.
5. Safety and Security:
- I will follow all safety guidelines and procedures while using the facilities and equipment.
7. Compliance with Policies:
- I will adhere to all policies and procedures set forth by North Sevier Recreation regarding the use of equipment and facilities.
I, as a North Sevier Rec Athlete, have read and understand the terms of this Equipment and Facility Usage Agreement. I agree to uphold these responsibilities and expectations while participating in the North Sevier Recreation Youth Sports Program. I understand that failure to comply with this agreement may result in disciplinary action, including the potential loss of athlete privileges.
