Parental Consent Form

I, the undersigned, as the parent/legal guardian of the student named above, hereby give permission for my child to participate in all activities associated with the North Sevier Recreation Middle School Debate Team. This includes, but is not limited to, team meetings, debate tournaments, and team-building activities.

I release and hold harmless NORTH SEVIER RECREATION, its coaches, staff, volunteers, Recreational Directors, and any other affiliates associated with Recreational Department from any liability for any injury, loss or damage that my child may incur as a result of participation in the program. I accept and assume all risks associated with my child’s participation in the event.

I understand that my child is a representative of the NORTH SEVIER RECREATION and is expected to act appropriately. No bullying, foul language, or any other behaviors that would be deemed inappropriate will be allowed. If such behavior occurs, the discipline will follow those of the Code of Conduct and Anti-bullying Rules.

I acknowledge that my child is required to adhere to the team's code of conduct and behavior expectations, and I understand that failure to do so may result in disciplinary action, including removal from the team.

I grant permission for my child to be photographed and filmed during events related to the team. I understand that these photos and videos may be used for promotional purposes (including on social media, websites, and print materials) by North Sevier Recreation and its affiliates.

Youth Sports Code of Conduct Agreement

Introduction

As members if the North Sevier Recreation Middle School Debate Team, we are committed to fostering a respectful, supportive, and competitive enviroment where all participants can grow intellectually, socially, and emotionally. This Code of Conduct outlines the expectations for behavior and attitudes that will guide our team through the season.

  1. Respect for Others

  • Treat all team members, coaches, judges, and opponents with kindness, courtesy, and respect.

  • Listen attentively when others are speaking, and never interrupt or belittle others' opinions or arguments.

  • Embrace diversity of thought and encourage open-mindness and constructive dialogue.

  • Recognize that each person has the right to express their ideas without fear of ridicule or discrimination.

  1. Sportsmanship

  • Win graciously and lose with dignity. Handle both victory and defeat with respect toward your peers, coaches, and opponents.

  • Avoid negative commets or actions that may disrupt the atmosphere of hurt others' feelings.

  • Acknowledge the achievements of your teammates and competitors. Applaud others' efforts whether or not you agree with their opinions.

  1. Commitment to Preparation

  • Arrive on time and be fully prepared for practices, debates and team events.

  • Make the effort to stay informed about current events, relevant topics, and debate formats.

  • Be ready to engage in meaningful research and practice to continually improve your debate skills.

  • Respect the importance of individual and team preparation, recognizing that collective success is dependent on everyone's participation and effort.

  1. Integrity

  • Be honest and transparent in all aspects of debate, from research to argumentation.

  • Do not plagiarize or misrepresent the work or ideas of others.

  • Represent yourself, you team and North Sevier Recreation with integrity both on and off the debate floor.

  1. Teamwork

  • Support your teammates, both in their individual growth and as part of the collective team.

  • Offer constructive feedback and guidance in a supportive manner.

  • Help foster an inclusive and collaborative environment where every member feels valued and encouraged.

  • Share the responsibility of maintaining a positive, motivated, and cohesive team culture.

  1. Behavior During Events

  • Follow all event guidelines and rules set by event organizers, judges, and coaches.

  • Maintain focus during the competition and remain attentive to all parts of the debate.

  • Remain seated and respectful during speeches or presentations given by other teams.

  • Display exemplary behavior throughout the duration of any event, representing the North Sevier Recreation team and your school with pride.

  1. Responsibility

  • Take responsibility for your actions and decisions, both within the team and during competitions.

  • Notify coaches or team leaders in advance if you are unable to attend a practice or event.

  • Be accountable for your personal growth and development in the debate arena.

  1. Conflict Resolution

  • Handle conflicts or disagreements with maturity and professionalism.

  • Engage in peaceful, respectful discussions to resolve conflicts, and always seek to understand all perspectives.

  • If necessary, involve coaches or team leaders to mediate and assist with resolving issues.

  1. Communication

  • Keep open lines of communication with your coaches, teammates, and event organizers.

  • Ask questions when you need clarification, and be proactive in seeking help to improve.

  • Provide honest feedback when appropriate and use communication as a tool for growth.

  1. Personal Behavior and Health

  • Maintain a positive attitude and approach challenges with resilience and determination.

  • Prioritize health and well-being, and seek balance between academic responsibilities, debate, and personal life.

  • Ensure that your behavior at all times reflects well on the North Sevier Recreation program and your school community.

Enforcement

The North Sevier Recreation Debate Team coaches and leadership are committed to ensuring that this Code of Conduct is followed. Any member who violates the guidelines outlined in this document will be subject to review, and consequences may include temporary or permanent removal from the team.

Positive Example and Anti-Bullying Policy

This contract aims to establish a clear anti-bullying policy for ALL athletes and coaches. By signing this contract, I agree to abide by the outlined rules and understand the consequences of engaging in bullying behavior.

  • Zero Tolerance:
    Any Form of bullying, including physical, verbal and cyberbullying, will NOT be tolerated.

  • Respect:
    Athletes must show respect towards teammates, coaches, opponents, and any other individuals involved with the sporting program.

  • Inclusivity:
    I agree to promote a supportive and inclusive team environment for ALL members.

  • Reporting:
    I agree to report any incidents of bullying to the coach or a responsible authority immediately. (Parents, Recreational Director etc.)

Definition of Bullying:

  • Physical Bullying: Hitting, Pushing, or other forms of physical aggression.

  • Verbal Bullying: Name-calling, insults, or other derogatory comments

  • Cyber-bullying: harassing or threatening messages sent through digital platforms

  • Social Bullying: excluding someone from activities or spreading rumors.

Consequences for Bullying:

  • First Offense:

    • Verbal Warning from Coach.

    • Immediate suspension from one game

    • Written apology to the victim.

  • Second Offense:

    • Mandatory Meeting with the coach, parents, athlete and Recreational Department.

    • Suspension from three games.

    • Probationary period where the athlete's behavior is closely monitored.

  • Third Offense:

    • Expulsion from the team.

    • No Refunds Given.

As a North Sevier Athlete, I agree to the following:

  • I will be a role model for my peers, both on and off the field.

  • I will exhibit behaviors that reflect positively on myself, my team and my community.

  • I will not engage in any form of bullying, including physical, verbal, and cyberbullying.

  • I will promote a supportive and inclusive team environment.

  • I will report any bullying incidents, whether involving myself or something I have witnessed to the Coach or The Recreation Department.

By signing this contract, the athlete agrees to uphold these standards and understands the consequences of failing to do so.

Equipment and Facility Usage Agreement


As participant in the North Sevier Recreation Youth Club Program, you will have access to various facilities and equipment necessary for conducting practices and debates. This agreement outlines the responsibilities and expectations related to the use of these facilities and equipment.


Responsibilities and Expectations:


1. Proper Use of Equipment:

   - I agree to use all equipment provided by North Sevier Recreation in a safe and appropriate manner.

   


2. Maintenance and Care:

   - I will take responsibility for the proper care and maintenance of the equipment while it is in my possession.

   - I will report any damaged or malfunctioning equipment to the Club Coordinators or recreational director immediately.


3. Return of Equipment:

   - I agree to return all equipment in good condition at the end of the season or upon request by North Sevier Recreation.

   - I understand that I may be held financially responsible for any lost, stolen, or damaged equipment due to negligence.


4. Cleanliness and Order:

   - I will ensure that all facilities are left clean and orderly after use.

   - I will dispose of any trash and ensure that all personal and team items are removed from the facility.


5. Safety and Security:

   - I will follow all safety guidelines and procedures while using the facilities and equipment.

 


7. Compliance with Policies:

   - I will adhere to all policies and procedures set forth by North Sevier Recreation as well as by Sevier School District regarding the use of equipment and facilities.


Agreement:

I, as a participant  have read and understand the terms of this Equipment and Facility Usage Agreement. I agree to uphold these responsibilities and expectations while participating in the North Sevier Recreation Youth Club Program. I understand that failure to comply with this agreement may result in disciplinary action, including the potential loss of Club privileges.
