As an athlete participating in North Sevier Recreation Youth Sports, I agree to:
Treat teammates, coaches, officials, opponents, spectators, volunteers, Recreation staff, and all other individuals associated with the program with respect.
Play fairly and follow the rules of the sport.
Demonstrate good sportsmanship before, during, and after practices and games.
Attend practices, games, team meetings, and other required team activities on time and prepared to participate.
NotifyT & C my coach or parent/guardian as soon as possible when I am unable to attend a scheduled activity.
Listen to and follow reasonable instructions from coaches, officials, and Recreation staff.
Use appropriate language and behavior at all times.
Refrain from taunting, threatening, bullying, harassment, fighting, or other inappropriate conduct.
Respect teammates and opponents regardless of skill level, ability, background, or experience.
Take reasonable care of Recreation facilities, fields, courts, equipment, uniforms, and other property.
Report bullying, harassment, unsafe conditions, or other inappropriate behavior to a coach, parent/guardian, Recreation staff member, or another trusted adult.
Represent myself, my team, and North Sevier Recreation in a positive manner both during and outside of team activities.
As a coach participating in North Sevier Recreation Youth Sports, I agree to:
Treat all athletes, parents/guardians, officials, opponents, spectators, volunteers, Recreation staff, and others with respect and fairness.
Serve as a positive role model by demonstrating good sportsmanship, integrity, patience, and appropriate behavior.
Provide a safe, supportive, and inclusive environment in which athletes can learn and participate.
Encourage teamwork, discipline, effort, respect, and personal responsibility.
Follow all North Sevier Recreation rules, policies, procedures, and sport-specific guidelines.
Communicate expectations, schedules, changes, and other important information clearly and appropriately.
Address bullying, harassment, unsafe behavior, and other inappropriate conduct promptly and report serious or repeated incidents to North Sevier Recreation.
Maintain appropriate boundaries with athletes and communicate with athletes in a professional and age-appropriate manner.
Refrain from physical contact with athletes except for appropriate, reasonable, and necessary contact related to instruction, safety, or emergency circumstances.
Never use physical punishment, intimidation, threats, humiliation, or abusive language as a form of discipline.
Refrain from aggressive, threatening, or confrontational behavior toward athletes, parents/guardians, officials, opponents, or Recreation staff.
Follow all concussion, injury, emergency, and safety procedures established by North Sevier Recreation.
Report injuries, serious incidents, or other situations requiring Recreation involvement as soon as reasonably possible.
Understand that coaching is a position of responsibility and that North Sevier Recreation may remove or restrict a coach's participation when necessary to protect athletes or the integrity of the program.t
As a parent or guardian of a North Sevier Recreation athlete, I agree to:
Encourage my child to follow the rules and demonstrate good sportsmanship.
Respect coaches, officials, opponents, spectators, Recreation staff, volunteers, and other participants.
Support my child's participation by ensuring that my child arrives on time, prepared, and appropriately equipped.
Notify the coach when my child will be absent or unable to participate.
Allow coaches to coach and officials to officiate.
Refrain from coaching my child or other players from the sidelines during games or practices unless specifically asked to assist by the coach.
Refrain from yelling at, criticizing, threatening, or confronting officials, coaches, athletes, or other spectators.
Refrain from using profanity, abusive language, threatening gestures, or other disruptive behavior.
Address concerns or complaints through the appropriate communication process and in a respectful and constructive manner.
Avoid confronting a coach immediately before, during, or immediately after a game when emotions are elevated. When possible, concerns should be discussed at an appropriate time.
Promote a positive and supportive atmosphere for all athletes.
Report bullying, harassment, threats, unsafe behavior, or other serious concerns to the coach or North Sevier Recreation.
Respect restricted areas of facilities, including team areas, playing areas, locker rooms, officials' areas, and other areas designated by Recreation staff.
Follow all facility rules and North Sevier Recreation policies.
Spectators attending North Sevier Recreation Youth Sports activities are expected to:
Encourage athletes rather than criticize them.
Respect officials and their decisions.
Respect opposing teams and their spectators.
Refrain from profanity, threats, harassment, or abusive behavior.
Remain in designated spectator areas when requested.
Follow facility rules and staff instructions.
Never enter a playing area without authorization unless there is an emergency.
Support an environment in which children can enjoy learning and participating in sports.
North Sevier Recreation reserves the right to remove a spectator from a facility or event for inappropriate, disruptive, threatening, or unsafe behavior.
Violations of this Code of Conduct may result in disciplinary action.
North Sevier Recreation will consider the nature, severity, frequency, circumstances, and impact of the conduct when determining an appropriate response.
Depending upon the circumstances, disciplinary action may include:
Verbal warning.
Written warning.
Meeting with the athlete, parent/guardian, coach, and/or Recreation staff.
Removal from a practice, game, or activity.
Suspension from one or more practices, games, or activities.
Probation or behavioral agreement.
Removal from a team.
Suspension from participation in a Recreation program.
Removal from a Recreation facility or event.
Suspension or removal of coaching privileges.
Permanent or temporary restriction from participating in or attending Recreation activities.
Referral to the North Sevier Recreation Director or designated disciplinary authority for further action.
North Sevier Recreation may impose an immediate suspension or removal when conduct is severe enough to create a safety concern or significantly disrupt a program.
Disciplinary action may be taken for conduct occurring during a Recreation activity or, when appropriate, conduct occurring outside of a Recreation activity that directly impacts the safety, operation, or integrity of the program.
North Sevier Recreation reserves the right to determine the appropriate disciplinary response based on the circumstances of each situation.
North Sevier Recreation is committed to providing a safe, respectful, and inclusive environment for every athlete.
Bullying, harassment, hazing, threats, intimidation, and other behavior intended to harm, humiliate, intimidate, or exclude another participant are prohibited.
North Sevier Recreation does not tolerate:
Physical bullying or aggression.
Verbal bullying or harassment.
Threats or intimidation.
Cyberbullying.
Hazing.
Deliberate and repeated exclusion intended to harm or humiliate another participant.
Spreading harmful or malicious rumors.
Harassment through social media, text messages, or other electronic communication.
Retaliation against an individual who reports a concern or participates in an investigation.
For purposes of this policy, bullying generally involves repeated or severe behavior that is intended to harm, intimidate, humiliate, threaten, or exert control over another person.
Bullying may include, but is not limited to:
Hitting, kicking, pushing, tripping, or other intentional physical aggression.
Repeated insults, name-calling, threats, degrading comments, or other abusive language.
Deliberately and repeatedly excluding or isolating another participant, spreading harmful rumors, or attempting to damage someone's relationships or reputation.
Using text messages, social media, online platforms, or other electronic communication to threaten, harass, humiliate, or intimidate another person.
Any activity or behavior required, encouraged, or permitted as a condition of joining, remaining on, or participating with a team that humiliates, degrades, threatens, or endangers another person.
Normal disagreements, occasional conflicts, competitive behavior, or reasonable coaching and correction do not necessarily constitute bullying. North Sevier Recreation will consider the circumstances and nature of the conduct when determining whether behavior violates this policy.
Athletes, parents/guardians, coaches, officials, and spectators are encouraged to report suspected bullying, harassment, hazing, or threats.
Reports may be made to:
The athlete's coach;
The Sport Commissioners;
The North Sevier Recreation Director;
Another North Sevier Recreation staff member; or
A parent/guardian or trusted adult who can assist in making a report.
Reports involving immediate threats or danger should be reported to emergency services or law enforcement when appropriate.
North Sevier Recreation may review and investigate reported incidents.
When appropriate, the Recreation Department may:
Speak with the individuals involved.
Speak with witnesses.
Review available information or documentation.
Consult with coaches, parents/guardians, Recreation staff, or other appropriate individuals.
Take temporary action to protect participants while an investigation is conducted.
North Sevier Recreation will make reasonable efforts to handle reports appropriately and maintain confidentiality to the extent reasonably possible. However, complete confidentiality cannot always be guaranteed.
False or intentionally misleading reports may also result in disciplinary action.
Retaliation against an individual who makes a good-faith report or participates in an investigation is prohibited.
Any participant, coach, parent/guardian, or other person who retaliates may be subject to disciplinary action.
Bullying and harassment are serious violations of the North Sevier Recreation Code of Conduct.
Depending upon the severity and circumstances of the incident, consequences may include:
Verbal or written warning.
Parent/guardian notification.
Meeting with the athlete, parent/guardian, coach, and Recreation staff.
Behavioral or probationary agreement.
Removal from a practice or game.
Suspension from one or more games or activities.
Removal from the team.
Suspension from Recreation programs.
Permanent removal from a Recreation program or facility.
North Sevier Recreation is not required to follow a specific sequence of disciplinary action. Serious conduct may result in immediate suspension or removal.
Refunds are not guaranteed when a participant is suspended or removed because of a violation of this policy.
North Sevier Recreation is committed to protecting athletes who may sustain a concussion or traumatic head injury.
This policy is intended to comply with applicable Utah requirements and establish procedures for recognizing, responding to, and managing suspected concussions and traumatic head injuries.
Utah law requires an amateur sports organization to adopt and enforce a concussion and head injury policy, provide the policy to a parent or legal guardian, and obtain acknowledgment before allowing a child to participate.
A concussion is a type of brain injury that may occur after a blow, bump, or impact to the head or after a forceful impact to another part of the body that causes movement of the head.
A concussion can affect the way the brain normally functions.
A concussion may occur with or without loss of consciousness.
Symptoms may appear immediately or may develop hours or days after an injury.
Possible symptoms may include:
Headache.
Dizziness.
Confusion.
Nausea or vomiting.
Balance problems.
Sensitivity to light or noise.
Difficulty concentrating.
Memory problems.
Fatigue.
Changes in behavior or mood.
Unusual sleepiness.
Other changes in normal behavior or functioning.
If a coach, Recreation staff member, official, volunteer, parent/guardian, or other responsible individual reasonably suspects that an athlete has sustained a concussion or traumatic head injury:
The athlete must be removed from participation immediately.
The athlete may not return to the game, practice, or other sporting activity until the requirements of this policy have been satisfied.
Utah law requires immediate removal when a child is suspected of sustaining a concussion or traumatic head injury.
When an athlete is removed because of a suspected concussion or head injury:
The parent or legal guardian should be notified as soon as reasonably possible.
The athlete should be monitored and provided appropriate first aid or emergency care as necessary.
The parent/guardian should be advised to seek evaluation from a qualified health care provider trained in the evaluation and management of concussions.
If emergency medical attention is warranted, emergency medical services may be contacted.
An athlete who has been removed because of a suspected concussion or traumatic head injury may not return to participation until the athlete:
Has been evaluated by a qualified health care provider who is trained in the evaluation and management of concussion; and
Provides North Sevier Recreation with a written statement from the qualified health care provider stating that the provider meets the applicable training requirements and that the athlete is cleared to resume participation.
Utah law specifically requires written clearance meeting the statutory requirements before an athlete may return to participation.
A coach, parent/guardian, athlete, or Recreation employee may not independently override this requirement.
An athlete removed from participation because of a suspected concussion or traumatic head injury will not return to the same game or practice without the required medical evaluation and written clearance.
When there is uncertainty, the athlete will remain out of participation.
When reasonably possible, the coach or Recreation staff member should document:
Date and time of the injury.
Location and activity in which the injury occurred.
How the injury occurred.
Symptoms observed or reported.
Whether there was loss of consciousness.
Whether there was memory loss or confusion.
Any first aid or emergency treatment provided.
Any medical instructions or clearance received.
By signing the North Sevier Recreation Youth Sports registration and acknowledgment documents, the parent/legal guardian acknowledges that they have received, read, and understand this Concussion & Head Injury Policy.
The parent/legal guardian understands that an athlete suspected of sustaining a concussion or traumatic head injury will be removed from participation and will not be permitted to return until the applicable medical clearance requirements have been satisfied.
North Sevier Recreation provides equipment and facilities necessary for youth sports programs. Participants, coaches, and teams are expected to use all Recreation property safely and responsibly.
Participants and coaches agree to use Recreation equipment only for its intended purpose and in a safe and appropriate manner.
Equipment may include, but is not limited to:
Balls.
Nets.
Goals.
Cones.
Training equipment.
Protective equipment.
Practice equipment.
Uniforms and team equipment.
Other Recreation-owned property.
Participants and teams are expected to take reasonable care of all equipment provided to them.
Any damaged, missing, or malfunctioning equipment should be reported to the coach or North Sevier Recreation as soon as possible.
Participants should not attempt to repair Recreation equipment unless authorized to do so.
All Recreation-owned equipment must be returned at the end of the season or whenever requested by North Sevier Recreation.
Participants or teams may be held responsible for equipment that is intentionally damaged, lost, stolen, or damaged because of negligent or unauthorized use, subject to applicable law and Recreation policies.
Participants, coaches, parents/guardians, and spectators are expected to help maintain clean and orderly facilities.
Individuals using Recreation facilities should:
Dispose of trash properly.
Keep equipment in designated areas.
Remove personal belongings after use.
Follow posted facility rules.
Report unsafe conditions or damage to Recreation staff.
Respect floors, walls, doors, bleachers, restrooms, fields, courts, and other facility property.
All participants and guests must follow safety instructions provided by North Sevier Recreation staff, coaches, officials, and facility personnel.
Individuals may not enter restricted areas without authorization.
Tampering with safety equipment, emergency equipment, building systems, or facility security equipment is prohibited.
All participants, coaches, parents/guardians, and spectators are expected to follow North Sevier Recreation policies and reasonable instructions from Recreation staff.
Failure to follow facility or equipment policies may result in removal from the facility, loss of participation privileges, or other disciplinary action.
I acknowledge that I have received, read, and understand the North Sevier Recreation Youth Sports Policies & Participant Agreements.
I understand that participation in North Sevier Recreation Youth Sports is a privilege and that athletes, coaches, parents/guardians, and spectators are expected to follow the rules and standards established by North Sevier Recreation.
I understand and agree to comply with:
The Youth Sports Code of Conduct.
The Anti-Bullying, Harassment & Hazing Policy.
The Concussion & Head Injury Policy.
The Equipment & Facility Use Policy.
Applicable sport-specific rules and Recreation policies.
I understand that violations of these policies may result in disciplinary action, including suspension or removal from a team, program, facility, or other Recreation activity.
I understand that North Sevier Recreation may take immediate action when necessary to protect the safety and well-being of participants, staff, coaches, officials, or others.
The following waiver should be reviewed and approved by the appropriate city/county attorney, risk-management representative, or other qualified legal counsel before being adopted or used as an official North Sevier Recreation waiver.
I understand that participation in North Sevier Recreation Youth Sports involves inherent risks of injury.
These risks may include, but are not limited to:
Falls.
Collisions.
Contact with other participants.
Contact with equipment.
Sprains and strains.
Broken bones.
Head injuries and concussions.
Illness.
Serious injury.
Permanent disability.
Paralysis.
Emotional or psychological injury.
Property damage.
In rare circumstances, death.
I understand that injuries may occur even when reasonable safety precautions are taken.
I voluntarily choose to allow myself or my child to participate in North Sevier Recreation Youth Sports and acknowledge the risks associated with participation.
To the extent permitted by applicable law, I, on behalf of myself and, where applicable, my minor child, heirs, personal representatives, and assigns, agree to release and hold harmless North Sevier Recreation and the applicable governmental entity, together with their officers, employees, agents, volunteers, representatives, and other authorized personnel, from claims arising from participation in the program to the extent such release is permitted by Utah law.
This waiver does not waive or release any claim or liability that cannot legally be waived or released under applicable law.
In the event of an emergency, I authorize North Sevier Recreation personnel to provide reasonable emergency assistance and to contact emergency medical services when deemed necessary.
I understand that Recreation staff are not acting as my child's personal medical providers.
I understand that emergency medical transportation or treatment may result in costs for which I may be responsible.
I agree to provide accurate emergency contact and medical information to North Sevier Recreation and to update that information when necessary.
I understand that participation in youth sports involves physical activity.
I agree to inform North Sevier Recreation of any known condition or limitation that may affect my child's ability to safely participate.
I understand that North Sevier Recreation may restrict or discontinue an athlete's participation when staff reasonably believe continued participation may create a safety concern.
I understand that participants are expected to use Recreation facilities and equipment appropriately.
To the extent permitted by applicable law, I acknowledge responsibility for damage to Recreation property caused by my or my child's intentional, reckless, or negligent conduct.
This agreement shall be interpreted under the laws of the State of Utah, to the extent permitted by applicable law.
If any provision of this agreement is determined to be unenforceable, the remaining provisions shall remain in effect to the extent permitted by law.
I certify that I have carefully read this Waiver & Release of Liability and understand that participation in North Sevier Recreation Youth Sports involves risks.
I understand that I am voluntarily permitting myself or my minor child to participate.
I understand that I have had the opportunity to ask questions regarding this agreement before signing.
